The form is long because a proper review needs detail. Complete what applies and leave the rest — partial submissions are fine and we will come back on the gaps.
Documents attached here are stored in our secure client record system, not on this website. Please do not send FDA or CBP portal passwords, government credentials, banking details or Social Security numbers — we will never ask for them.
What gets checked
An entry review reduces avoidable problems. It cannot guarantee clearance, release or admissibility — CBP and FDA hold sole authority over entry, examination, sampling, detention, refusal and admissibility decisions, and government timelines vary.
If the cargo has landed, do not use this form
A held shipment needs a phone call, not a review queue. Storage and demurrage accrue daily.
